rename primary key index when renaming table in postgresql

postgresql does not rename indexes associated with a table when you ALTER a table name.
This small patch renames the primary key index that django always produces when creating a table in an analogous fashion to the way the sequence gets renamed
- Status changed from new to assigned
- Version set to 0.3
- Milestone set to 0.4
